New Patients
Complete and sign Pages 1-8 and bring those completed pages with you to your first appointment.
The remaining pages contain our Notice of Privacy Practices for your review.
You do not need to bring the Notice of Privacy Practices pages back to our office. You may keep them for your records. The Notice of Privacy Practices explains how your health information may be used and disclosed and describes your rights regarding your health information.
Established Patients
If it has been 1-3 years since your last visit, please print, complete, and bring our Established Patient Update Packet to your appointment.
If it has been more than 3 years since your last visit, please complete the New Patient Packet instead.
Notice of Privacy Practices
Please bring:
- Your current insurance card(s)
- Photo identification
- Any imaging, test results, or medical records our office has specifically asked you to bring
Insurance Plans Accepted
